We recognize that the consultant has a critical role in long term care. Our Pharmacists provide drug regimen reviews for each resident on a monthly basis, evaluating for drug to drug interactions, drug to food interactions, and appropriateness of dosing and disease state management. The Pharmacist thus helps coordinate resident care with the physicians and staff to improve patient outcomes. The goal of this team approach is to improve residents’ quality of life and decrease hospital admissions.
Our Consultant Pharmacists offer expertise and educational programs in pharmaceutical care for your facility staff. We reiterate that our Consultants will be active members of your healthcare team. HomeTown Pharmacy LTC consultants are not independent contractors. Our consultants are required to work in-house on dispensing functions on a routinely scheduled basis. This allows us to efficiently coordinate dispensing and consulting functions resulting in quick resolution of facility concerns.
