
HomeTown Pharmacy LTC staff will oversee and implement all facets of the conversion process and make the transition to our pharmacy quick, efficient and worry free. Upon establishment of a start date, the entire conversion process, including staff in-servicing, preparation and transfer of medical records, switching out medication carts, and communication with resident’s families will be performed by our staff.
On site Family nights, and other meetings with staff and residents, will be performed if deemed necessary to ensure a smooth transition. In addition, before, during and after the transition our customer service representative will be on site regularly, addressing questions or bringing in the appropriate staff members to do so. We are proud of our transition process, and our facilities will attest to the fact that we execute the changeover with precision and efficiency
